Investing in Community Health
Health institutions and community investment practitioners share the goal of helping people live healthy lives. There are many ways to promote community health, but one of the most powerful is investing in the social determinants of health: housing, jobs, education, food security, and health care. When health institutions bring their assets—land, financial resources, relationships, expertise—to community investment partnerships, everyone benefits.
Creating Healthy Communities
Healthy communities have thriving community investment systems. Community and health leaders can advance individual deals and projects and local systems by bringing in new investors, improving local investment and building policies, and increasing the scale of their work. This lays the groundwork for sustaining community health over the long term.

Agents of Change
Making health equity a reality involves eliminating health disparities so that individuals have equal opportunities to be healthy.
The Healthy Housing, Healthy Communities (H3C) initiative, a partnership with The National Council of State Housing Agencies (NCSHA), is assisting state housing finance agencies to develop strategic partnerships with hospitals and other health organizations to finance affordable housing.
Past Programs
Hospitals and health systems can be game-changing participants in efforts to improve conditions in the places they serve. They can do this through investing upstream in the social determinants, harnessing an array of assets to create more equitable, sustainable, and healthy communities.
The Health Partner Investment Action Lab, a partnership with NeighborWorks America which launched in 2021, is assisting community development organizations to partner with health institutions on upstream investments in affordable housing.

Institutions are committed to improving the health of the communities they serve by working with other sectors to address the factors that influence health, including employment, housing, education, health care, public safety, and food access.
